Insurance Information

If you have insurance, we will contact them for benefits prior to your appointment. Please also feel free to contact them for verification. You will be responsible for your estimated co-insurance portion at the time of service. Insurance pre-estimates are not a guarantee of payment. If you have a balance after insurance has remitted payment, a statement will be mailed to your home.

Q.  Will this be covered by my dental insurance or medical insurance?

A.  If a covered benefit, procedures involving the teeth and gums may be covered by dental insurance.  All other procedures, if a covered benefit, may be covered by your medical insurance.  If the proper insurance information  has been provided to our office before your first visit, we will give you an estimate of insurance benefits at the time of your first visit.

If we are not providers under your plan AND you do not have out-of-network benefits, the cost of surgery is due at the time of service.

We are currently contracted with the following dental plans:

• Delta Dental (Premier Provider)

We accept all out-of-network benefits that your dental insurance coverage may provide.

We are currently contracted with the following medical plans:

• Blue Cross/Blue Shield of Arizona (PPO)
• Medicare

We accept all out-of-network benefits that your medical insurance coverage may provide.
