New Patients

We look forward to meeting with you and hope you have a confident and comfortable experience here at AZ Maxillofacial Surgeons P.C. Your initial appointment will consist of a consultation discussing your diagnosis, treatment options, and financial responsibilities. Consultations allow you and the surgeon to review medical information crucial to the success of your procedure.

IMPORTANT:  All patients under the age of 18 must be accompanied by a parent or legal guardian during their visit to our office.

Please assist us by providing the following information at the time of your consultation appointment:

• Any referral slip and X-rays that your dentist may have provided for you
• A list of all medications you are currently taking
• Dental and/or medical insurance information, if applicable
• Consultation fee (We accept Visa, MasterCard, Discover, American Express, cash, or checks)

Be prepared to arrive 15 minutes before your scheduled appointment to complete paperwork.  You may fill out our online forms 48 hours prior to your appointment.
